Our Foundation could not be effective
if it wasn't for the generous donations of others.
ESTABLISH A MEMORIAL OR HONORARY FUND WITHIN THE FOUNDATION: Unfortunately, with this disease we experience several losses throughout
the year. It is through the caring and sharing of these families that lose a loved one, they opt to establish a "fund"
within our foundation to honor their loved one and to raise necessary dollars to further research. Many, many, thousands
of dollars are generated each year through memorial gifts. If you would like to establish a memorial fund for a loved one we would be honored to assist you
with that and can assure you that EVERY dollar that is donated will go directly towards LMS research. You may establish
a memorial or honorary fund in a loved ones name by contacting Lori Hoag, the foundation secretary/ treasurer, at lori@nlmsf.org or by phoning the foundation at (607) 638-9551. All donations are tax deductible under our 501 C-3 non profit organization status. Each
donor will receive a personal letter of thanks and gratitude from the foundation as well as a receipt for their tax records. Family members are also notified of each donation that is
received in honor of their loved one. As long as donors notate the honoree's name on the payment, their family will
be notified that a gift has been made in their name. This is also a wonderful way to honor LMS Survivors for special

in their honor. Please specify a name with your donation. Anyone may contribute to these funds at any time. The NLMSF will
contribute to LMS research on their behalf. First and last names will only be listed on this site with permission. The loved
ones are listed in alphabetical order (when applicable), followed by the name of the fund. |
